- Download the application form in PDF format.
- Fill up the application form and the feeslip (provided at the last page of the application form).
- Submit the fee (in cash) along with the filled up feeslip to any branch of ICICI Bank. The bank will stamp and return the ASCL COPY and the STUDENT COPY of the feeslip to you.
- Submit the filled-in application form along with the ASCL COPY of the feeslip at:
